Adult Drivers Ed Requirements

Hello Driver’s Education Registrants,

The following required forms are necessary in order for you to take your driving test at the end of this program. These forms will be available to you at check-in. It will save you time if you fill them out (IN BLACK INK) before you come - IMPORTANT: YOU MUST NOT SIGN THEM!  - these documents must be signed in the presence of a Notary Public; TSD will provide this service for free at check in.

  • DL-14A Application for Texas Driver License or Identification Card for 17 years old 10 months of age or older

    DL-5 Texas Residency Affidavit

    • When you sign this form in the presence of the Notary Public at TSD, the signature MUST match the name that is on the proof of residency (see below).  Either mother or father or guardian signature and address must be the same on all these forms: Proof of Residency, Texas Residency Affidavit, DL-14A / B. Also, that same person must be present with the student at time of check-in.

    • Two documents proving residency are required.  Please see the following samples of what will be accepted at DPS. If using an electric, water, natural gas, satellite television, cable television, or non-cellular telephone statement must be dated within 90 days of the date of application (or the first day of Summer Programs).

The Department of Public Safety requires the following documentation.  These must be left with us at TSD as DPS will not accept copies.  For identification protection, TSD will lock up these documents for safe-keeping.  The documents will be returned to you at the end of Summer Programs.

  • Student's ORIGINAL social security card

  • Student's ORIGINAL birth certificate

  • Student's VALID Texas ID or ORIGINAL school photo ID or VALID passport

  • Physician’s statement (signed by your doctor)

Finally, the you must arrive at check-in with:

  • Prescription glasses/contacts for the vision screening exam that you will take at the Department of Public Safety. Important note:  All students must take a vision exam at DPS.  We strongly suggest that your vision be tested prior to arrival for Summer Programs.  If you fail the vision exam at DPS, your parent/guardian will be responsible for making an eye exam appointment during summer programs so you can complete the course.
